To the parents/guardians of Dream It, Be It Participants:

SI-Las Cruces must have on file a consent form signed by a parent or guardian of each conference participant under the age of 18. Please complete and return one of the forms below prior to the start of the conference on April 9 at 8:30 am.

Box 714, Las Cruces, NM 88004 or have the participant return it to the registration desk the morning of the conference.
